Discharge Support

Discharge Support | Destin, FL | Aqua Home Care

Your Partner in Recovery

Leaving the hospital or rehabilitation center is only the first step toward recovery. Our Discharge Support service in Destin, Florida, ensures your loved one transitions home with confidence, safety, and the right level of care. We’re here to make sure that homecoming is not overwhelming, but instead, the beginning of a smooth healing journey.

Recovering after a hospital stay can be challenging, physically, emotionally, and even logistically. Our compassionate team is dedicated to removing those challenges by creating a discharge plan tailored to your loved one’s unique needs. We understand that the days following discharge are often the most crucial, which is why we work closely with you, your loved one, and medical professionals to ensure every detail is taken care of.

An Aqua Home Care Area Manager will meet with you before discharge to assess the specific needs of your loved one. Whether they require assistance with mobility, medication reminders, meal preparation, or personal care, our team ensures nothing is overlooked. We coordinate with doctors, nurses, and therapists so that care at home matches the medical guidance provided during recovery.

Our Discharge Support is about more than simply getting someone home; it’s about creating an environment where they can heal safely and comfortably. This includes fall prevention strategies, transportation assistance, post-operative care, or simply having a kind and attentive caregiver nearby to provide reassurance. Our caregivers are highly trained, screened, and prepared to respond to the evolving needs of your loved one.

Recovery doesn’t follow a strict schedule, which is why an Aqua Home Care team member is available 24 hours a day, 7 days a week. You can rely on us for both planned support and urgent needs, ensuring that help is always just a call away.

With our Discharge Support service, your loved one comes home with a trusted care partner by their side. From the moment they leave the hospital to the day they feel fully themselves again, we’ll be there every step of the way.

This is my first experience with private duty nursing and it has been awesome. Mike and his team are beautiful to work with. They matched me with Lod who is a beautiful person inside and out. I love her and wish I could adopt her. She is kind, respectful, considerate and has been a true blessing. Thanks guys. You are all the BEST!!!

The service and the care that we’ve gotten from Brian has been far above anything else we’ve had in the almost 2 years of home care that we have had. I make it difficult for them with a flexible schedule that I need and they’ve always been able to accommodate us.

Aqua Healthcare has been a godsend. I know that I couldn’t have taken care of my 92 year old father without their skillful and kind care. They were there at the end of my fathers life when I couldn’t be.
